Please list the full names of all people (including any siblings under 18) who are authorised to collect your child / children at any time without us needing to call you for confirmation. Example: “Jane Smith – Grandma, John Smith – Uncle.”

Please list the full names of any people (including any siblings under 18) who may collect your child/children but will require a verification phone call to the account holder every time before we release your child / children. Example: “Sarah Jones – family friend (call required).”
